MEASLES  (Rubeola)

 

᎙ Agent factors:  RNA paramyxovirus

 

᎙ Period of communicability:  4-5 दिन पहले और दाने के 4-5 दिन बाद।

 

Host factors:

                                    ᎙ सबसे आम उम्र: 6 महीने से 3 साल।

                                    ᎙ (>विकासशील देशों में 5 वर्ष)

 

Environmental factors:

        1. Season: Any season (spring pressure)

        2. Mode of transmission:  Airborne, छोटी बूंद, droplet nuclei.

 

Clinical features:

                        इन फीचर्स को तीन तरह की स्टेज में डिवाइड किया जा सकता है: 

   

    1- Prodromal stage

    2- Eruptive stage

    3- Post measles stage

 

Prodromal stage:  

                                                        बुखार, जुकाम, malaise, koplik spots (छोटे, सफेद धब्बे  (often on a reddened background) जो measles के शुरुआती दौर में गालों के अंदर होते हैं।)

 

Eruptive stage:

                        ᎙  Rash behind ear, face to body.

                        ᎙  दाने (Rash) T - कोशिकाओं के कारण होते हैं जो वायरस से संक्रमित होते हैं

 

Post measles stage: कमजोरी/दस्त

 

Complications of measles:

        ᎙ Diarrhoea (सबसे आम संबंधित विशेषता)

        ᎙ Pneumonia (मृत्यु का सबसे आम कारण)

        ᎙ Otitis media ( most common complication )

 

Treatment:

        ᎙ ORS

        ᎙ Vitamin A prophylaxis

        ᎙ Supportive treatment

 

› Live attenuated vaccine:  Edmonston zagreb strain 

 › 0.5 ml: >1000 infective virus units

› Subcutaneous at right upper arm (0.5 ml)

› Use within 4 hours of reconstitution

›  Immunity with measles vaccine develops after 10-11 days 

 › अतिसंवेदनशील संपर्कों के मामले में खसरे का टीका संपर्क के 3 दिनों के भीतर दिया जाना चाहिए ।

 

    राष्ट्रीय टीकाकरण schedule के अनुसार Measles का टीका लगाने का सही समय

 

        1- खसरे का पहला टीका → 9 महीने

           2- खसरे के टीके की दूसरी खुराक → 16 - 24 महीने
